PREREQUISITES AND PLACEMENT IN THE CURRICULUM:
Surgery and Internal Medicine core clerkships
PURPOSE:
The aim of this elective is to provide students with adequate knowledge of and comfort with the diagnosis and management of acute and chronic liver diseases and their subsequent complications. Basic physiologic functions of the liver and the approach to the diagnosis of liver diseases will be emphasized; case discussions/didactics will focus on viral hepatitis, metabolic and autoimmune liver diseases, cirrhosis and its complications, hepatocellular carcinoma, liver transplantation, and rarer types of hepatobiliary disorders. As the burden of liver diseases steadily increases, familiarity with liver pathology is critical to the practice of medicine across specialties.
LEARNING OBJECTIVES:
- List the etiologies of hepatobiliary diseases; contrast and distinguish their respectiveclinical manifestations and pathophysiology; propose and evaluate the diagnostic andmanagement strategies needed to address them.
- State the indications, contraindications, and selection process for liver transplantationand describe the approach to immunosuppression following transplantation.
- In preparation for internship, to demonstrate competence in approaching patients withmedical complexity, additionally to propose and justify management decisions in saidpatients.
COMPETENCIES:
Students will be evaluated on their ability to:
- Actively participate in the evaluation of patients with liver disease in both the inpatient and outpatient setting
- Demonstrate a basic fund of knowledge in hepatobiliary disease
- Present patient cases in a clear and concise manner
- Advance knowledge during the clinical elective
INSTRUCTIONAL FEATURES:
The students will spend the majority of their time with the inpatient hepatology team, and will be responsible for following 1-2 patients per day. Responsibilities will include direct patient contact and presenting the patient to the attending physician on daily rounds. Additionally, the students will be expected to observe endoscopies performed on patients on the inpatient hepatology service. The students will also actively participate in 1-2 outpatient hepatology clinics per week with the attending physician on service. If interested, students can also observe one of the liver transplant clinics on Tuesday or Thursday mornings. The students will also be expected to participate in the Monday morning clinical case conference, weekly didactics given by the attending and fellows, as well as the Friday morning conferences: multidisciplinary Radiology conference (Tumor Board); pathology conference; and the weekly liver transplant listing meeting. A curriculum, including a library of articles and a list of educational resources, comparable to that provided to the residents, will also be given to the students at the onset of the rotation. The entire rotation will be supervised by residents, gastroenterology fellows, and/or hepatology attendings.
ASSESSMENT:
The attending physicians will provide individual verbal feedback at the mid-point and conclusion of the rotation. A composite written evaluation will be completed at the end of the month by the attending physicians.
