Fundamentals of Anesthesiology- UI Health

PREREQUISITES AND PLACEMENT IN THE CURRICULUM: Scrub and OR Etiquette Training and passed USMLE Step 1

NARRATIVE DESCRIPTION: 

  • Become familiar with the specialty of Anesthesiology.
  • Understand what anesthesiologists do in the operating rooms and outside the operating rooms.
  • The elective will help understand what the profession of anesthesiology does and how it relates to other specialties such as surgery, obstetrics, pain, pediatrics, and critical care.
  • M3 who are interested in pursuing anesthesiology as a potential career choice and are not sure.
  • The M3 student will learn the basics of anesthesiology such as pharmacology, physiology, and anesthesia equipment management.
  • Integrate into the team care model of patient care in anesthesiology.
  • The educational setting is fully in-person.

LEARNING OBJECTIVES:

At the conclusion of the course students will be able to:

  • Acquire basic knowledge in anesthesiology, which includes clinically applied physiology and pharmacology.
  • Integrate physical exam skills which will be used to identify risk factors for anesthetic complications.
  • Acquire the ability to interact and communicate with patients, families, and members of the anesthesia team which include the attending, resident or fellow, and CRNA.
  • Perform a comprehensive history and physical with supervision.
  • Identify general health issues that may affect anesthetic care.
  • Identify elements and process of informed consent.
  • Recognize common pain states and the treatment options for pain.
  • Recognize acutely ill or medically deteriorating patients.
  • Identify common acute events such as hypoxia, hypotension, and arrhythmias.
  • Participate in development of treatment plan and choice of anesthetic techniques.
  • Recognize airway patency and adequacy of ventilation based on clinical assessment.
  • Understand the correct use of standard monitoring devices.
  • Demonstrate knowledge of etiology, pathophysiology, diagnosis, and treatment of common medical and surgical problems.
  • Demonstrate commitment to patient care, emphasizing the best interests of the patient.
  • Start to display sensitivity and respect for the needs of diverse patient populations and the challenges of limited access to health care.
  • Comply with institutional policies and regulations.
  • Recognize the need to balance patient, personal, and institutional needs when providing health care.
  • Start to recognize situations where communication of information requires the assistance and asks for help.
  • Communicate effectively with respect of other members of health care team.
  • Start to communicate patient status to supervisor and to provide timely and accurate documentation under direct supervision.
  • Identify problems in quality of health care delivery.
  • Start to identify critical incidents or harmful events pertaining to the patient.

INSTRUCTIONAL FEATURES:

Description of Learning Activities

  • The length of the course is 2 weeks.
  • Learning activities include scheduled didactic lectures, lectures given by anesthesia faculty and residents in clinical situations, case reviews, morning conferences, grand rounds, simulations, and anesthesia management in the OR.
  • Didactic lectures will be for one hour on Monday, Wednesday, and Friday afternoons.
  • Morning conferences for 30 minutes on Monday, Tuesday, Thursday, and Friday. The morning conference may include subspecialty conferences.
  • Lectures by anesthesia personnel will be conducted during appropriate clinical situations in the operating room for about one hour daily.
  • Grand rounds will be for 1 to 1 ½ hour on Wednesdays.
  • Simulations will be conducted in the Anesthesiology Department.
  • Anesthesia management in the OR will consist of supervised activities such as airway management, IV access, preoperative assessment, postoperative care, pain management, monitoring, accurate record keeping, and dealing with events associated with anesthesia.
  • Students will be seeing and interacting with patients that are to have anesthesia for a surgical procedure. They may also be seeing patients on the pain management service and pain management clinic and the intensive care units.
  • Students will not be participating in any nights on-call unless they want optional extended time. Those that take call with the anesthesia team will not be expected to work the next day and may not get the same supervision and lecture opportunities.
  • Students will not have any weekend duties unless the student is making up a day or they want optional extended time.
  • The student will have clinical independence to review and assess patients in preparation of surgery. The student will have direct supervision by residents and faculty when anesthesia is being conducted.
  • Percentage of education that will be independent: 20%
  • Percentage of instruction directly supervised by residents: 60%
  • Percentage of instruction directly supervised by attendings: 20%

DAILY RESPONSIBILITIES: 

Normal work week will be a mixture of Anesthesia Management in the ORs, Didactic lectures, and Simulations. There will be some Lectures in the OR setting when appropriate. There will be hands-on training daily. The typical work hours per day will be 8 to 9 hours, for a total of 40 to 45 hours per week. Students are expected to read and study when not in the hospital.

Suggested text includes Pardo Jr., Manuel C., 8th ed. Miller’s Basics of Anesthesia. 2022. Which will be available to the student to borrow for the rotation.

ASSESSMENT

The student will be evaluated by residents and attendings by a standard evaluation form.

The student will be given feedback before the completion of the first week based on daily two way feedback and participation during the first week.
