Scleral Lenses in Current Ophthalmic Practice: an Evaluation

SCOPE (Scleral Lenses in Current Ophthalmic Practice: an Evaluation) is a multi-center, collaborative research group studying ocular disorders. The group, established in 2014, comprises national experts in ocular surface disease. The goal of our collaboration is to spearhead clinical research in scleral lenses, and to engage clinicians and scientists on a global scale.
IRB approval has been obtained at University of Illinois in collaboration with members of SCOPE Study Group. Donations will be used to fund a cross institutional study on ocular disorders including corneal and dry eye disease.

The SCOPE (Scleral Lenses in Current Ophthalmic Practice: an Evaluation) study group was established in 2014 under the auspices of the American Academy of Optometry Fellows Doing Research Special Interest Group. The group comprises national experts in the clinical fitting of scleral lenses who also have academic research interests. Because scleral lenses have only recently become part of mainstream clinical practice, there are significant gaps in our understanding of practice patterns, fitting philosophies and the biological effects of these devices. The goal of our collaboration is to spearhead clinical research in scleral lenses, and to engage clinicians and basic scientists on a global scale. Included in our mission the dissemination our findings to clinician, industry and patient stakeholders.
The Executive Committee members are: Jennifer Fogt, Jennifer Harthan, Amy Nau, Cherie B. Nau, Muriel Schornack and Ellen Shorter. We engage with a vetted network of other experienced scleral fitters interested in assisting with clinical and research projects on an ad hoc basis.

Summary of Past Research Activities
- SCOPE 1: An IRB approved, internet-based, 19 question survey was administered via email to 4,633 eye care providers. A total of 989 responses were collected representing a total of 84,735 patients. The survey was administered and analyzed by the Mayo Clinic Survey Research Center. Numerous posters and papers have been presented using data collected from this survey. Four manuscripts have been published in peer-reviewed journals (3 in Eye and Contact Lens and one in Journal of Contact Lens Research and Science) and a summary of results was published in Contact Lens Spectrum.
- SCOPE 2: An IRB approved, internet based, detailed practitioner survey regarding one patient experience that has been fit with scleral lenses in the past 6 months was administered via e-mail to 3,431 eye care providers. The SCOPE study team designed and administered the survey using REDCap. A total of 292 responses were collected representing 734 patient years of lens wear. Data have been presented in a number of posters. Two manuscripts have been published in Contact Lens and Anterior Eye.
- SCOPE 3: An IRB approved, patient-centric study enrolling keratoconus patients that was administered with the assistance of the National Keratoconus Foundation. A link to the survey was placed on the National Keratoconus Foundation website. The SCOPE study team designed and administered the survey using REDCap. A total of 425 responses were received. Data have been presented in a number of posters. One manuscript has been published in Optometry and Vision Science.
- SCOPE 4: An IRB approved survey of scleral lens fitting practices administered at the Global Specialty Lens Symposium, Las Vegas NV, January 2016. The SCOPE study team designed and administered the survey using REDCap. A total of 95 responses were received. Data have been presented in a number of posters. One manuscript has been published in Contact Lens and Anterior Eye.
- SCOPE 5: An IRB approved, patient-centric study enrolling patients with ocular surface disease that was administered with the help of several foundations that serve patients with severe ocular surface disease arising from a number of conditions. The SCOPE study team designed and administered the survey using REDCap. Data collection is complete and has been presented in a number of posters.
- SCOPE Scleral Lens Educator Survey: An IRB approved internet based survey of optometric educators. Data collection is complete. Data have been presented in poster format. A manuscript has been published in Contact Lens and Anterior Eye.
- SCOPE IOP Study: A clinical study evaluating the impact of scleral lens wear on intraocular pressure. Data collection is complete. Data have been presented in a number of posters and one manuscript published in Optometry and Vision Science.
- Keratoconus Practitioner Survey: An IRB approved internet based survey of eyecare providers. Data collection is complete. Posters will be presented at ARVO 2021.
- SCOPE Student Survey: An IRB approved internet based survey of optometric students. Data collection is complete. Data have been presented in a poster.
- SCOPE 2020: An IRB approved practitioner survey to assess current fitting practices, with attention given to changes in practice since the original survey was presented five years ago. Data collection is complete and data have been presented in a number of posters.
