Please note that the COM Registrar can only provide assistance to students enrolled in the College of Medicine. If you belong to any other college or require assistance regarding general degree or transcript matters, kindly visit the UIC Registrar website for further support.

Our department handles the registration/scheduling of current medical students and the education verification of MD graduates. Current medical students can make changes to their clinical schedule by submitting the Student Scheduling Request Form.
For all other inquiries and requests, e.g., education verification, please contact com-registrar@uic.edu.
Location
Our office is located at the College of Medicine West Tower in Suite 109, across from the Office of Student Affairs. We are open from 9:00am to 5:00pm, Monday through Friday. (Closed on Holidays.)

Requesting University Transcripts
Prior to requesting your transcript you should check your University Academic History, via the UIC Web for Student, to confirm that all of your grades are correct and have been posted.
If a grade and/or registration require a correction, please contact us at (312) 996-8228, and we will check your registration, to make sure it is correct, and we will email your grades to the University Office of Records to have them posted.
Once this is complete you can then request an official transcript from the University Office of Records.

- Complete the IL Certification of Graduation form, ED – MED
- Profession Name: Temporary Physician Licensure

- Email the completed ED – MED form to COM-Registrar@uic.edu.
- COM Registrar’s office will email a copy of the certified ED-MED to your uic.edu address.
- Order Official University Transcript verifying completion of your medical education to date, with the university seal affixed (see section below).
- Follow your program’s directions for submission usually two options:
- Submit both your ED – MED form and official university transcript directly to program,
OR - Upload your ED – MED form and official university transcript directly to IDFPR.

Note: For those who have different programs for PGY-1 and PGY-2: only the PGY-1 information to the state will be certified for you. Your PGY-2 GME Office has procedures for transferring temporary licenses. Contact that office no later than 60 days prior to the start date at your PGY-2. Submit documentation to IDFPR (Illinois Department of Financial and Professional Regulation)
Matched outside of the State of Illinois
- Request or receive documentation from your respective program, including a verification of medical education or medical school degree form.
- Check the date the form is due at the licensing board to allow sufficient time for processing. Ask your program if information is unclear or you have questions.
- Email completed form to COM-Registrar@uic.edu and note the date of the board’s deadline in the email subject heading.
- Note: Some states require this document to be submitted directly to the state licensing board by the College Registrar’s office.
- If the state requires a degree verification letter, please send an email to the Office of the Registrar at com-registrar@uic.edu. Include the name of the person to be addressed on the letter.
- If required to have the letter submitted with your complete residency packet, please indicate this in the email subject heading and the letter will be emailed to you.
- If the letter can be sent out directly from our office to the state licensing board, COM Registrar will send your letter as required.
- Order Official University Transcript verifying completion of your medical education to date, with the university seal affixed.
Proof of Medical Professional Liability Coverage
If you are leaving the University of Illinois for a residency and require proof of coverage please complete the request for “Proof of Medical Professional Liability Coverage” located at the Office of Business and Financial Services website.
Note: The certificate holder field on the request form requires the name and address of the outside entity requesting proof of coverage from the student. This section should not list the University of Illinois or any department there of as the certificate holder.

University Diploma Release Information
Diplomas are mailed to graduates whom all passing final grades have been received approximately 4 – 6 weeks after the term has ended.
If you have a Hold on your University record, your diploma will not be released.
University of Illinois diplomas are 8.5 x 11 inches and sent by first-class mail to a student’s diploma address approximately a week or two after your degree has been awarded. Please review or create a diploma address through student self-service as necessary before leaving the university
Once a diploma request is processed you will receive a secure ID by email within 48 hours. If you have received your diploma recently then you may already have this Secure ID in an email or with your actual diploma. The status of ordered diplomas can be viewed at this web site, Online Diploma Services.

M3 Clerkship Clinical Performance Reports
Processed by the COM Registrar’s Office. Evaluations are entered into an electronic web application by the Hospital Site Director and submitted to the Department Program Director at UIC. Once the completed evaluations is reviewed and approved, by the Program Director, they are submitted electronically to the COM Registrar’s Office for final processing. A final grade is calculated once a passing clinical grade and passing shelf exam score have been received by the COM Registrar’s Office. Your clinical grade, shelf exam score and final grade, once calculated, can be accessed via the College of Medicine On-line Score and Grade Summary application. A final grade letter and a copy of the clinical performance report is sent to you via USPS.
M4 Clerkship Performance Evaluation forms
Processed by the COM Registrar’s Office. Evaluations are received and processed daily. Final grades are entered into the College database and can be accessed immediately via the College of Medicine On-line Score and Grade Summary application. A copy of your evaluation is sent to you, via USPS.
Your mailing address (MA) should be kept up-to-date with the Student Self Service system.
The original copy of your evaluation is given to the Office of Student Affairs to be included in your academic file. If need to request a copy of an evaluation, submit a request via the Student Request Form (SRF) on the Student Affairs Office page. Click on Student Request Form (SRF).
IMPORTANT REMINDER —
Read the comments on all of your evaluations immediately after you receive them. If you have questions or concerns, address them immediately with the evaluator, Clerkship Hospital Site Director or Clerkship Program Director.
