Verification of Medical Degree/Certification Letters

If you have an application form for:

Requests for verification of graduation prior to 1994 require additional documentation that can only be provided by the University of Illinois at Chicago’s Main Campus, Office of Admissions and Records, which may delay the processing of your request.

Copy of Dean’s Letter or Medical Student Performance Evaluation (MSPE)

The College of Medicine Registrar’s Office does upload a copy of Dean’s Letter or Medical Student Performance Evaluation (MSPE) via Medical Institution Document Upload Service (MIDUS) to ERAS.

The College of Medicine Registrar’s Office does not produce or send University Transcripts. The University Transcript have to be ordered from the Main campus
University Registrar’s Office. You can request a copy of your University Transcript from that office and upload it independently to ERAS or have them sent directly to the program.

If you do not use ERAS and would like a copy of your Dean’s Letter or Medical Student Performance Evaluation (MSPE) to be sent to a fellowship program, you may contact The Office of the Regisrar, COM-Registrar@uic.edu. Please include the contact information for where the documentation should be sent.

Note: Copies of letters cannot be sent directly to the graduate.
