Student Request Form (SRF)
University of Illinois College of Medicine (UI COM) students can request specific documents and forms through the Student Request Form (SRF). Some documents require additional paperwork in order to complete your request. You may also complete a hard copy of the SRF at the Office of Student Affairs.
Use the below SRF and LOR fillable PDF forms to request the following items:
- Forms and Letters: Do not wait until the last minute to request letters from OSA. Make sure to allow plenty of time to submit your request. Most letters require 5 business days to complete. See the links below for a sample of the letters:
- Good Standing
- Good Standing with malpractice and health insurance coverage
- Jury Duty Exemption
- Financial aid loan deferment – students provide the form to be completed by OSA.
- Letter of Recommendation (a supplement to the SRF)
- Immunizations: If you are not a Castlebranch user to view the dates of your immunizations log into online student report.
Other Requests (do not use SRF)
- Transcripts must be ordered through the main UIC Admissions and Records Office.
- Alumni Verification of Medical Degree/Certification Letters can be requested through the College of Medicine Registrar’s Office.

UIC Medical Center Network Access (Powerchart)
- Read the Agreement; Sign at the bottom of the second page;
- Provide that original signature page to the Registrar in room 120, CMW.

Immunization & Health Certifications
Important note: Immunizations that are missing or expired must be uploaded to CastleBranch in a timely manner so that your record is current and up to date. This is a requirement for you to participate in college required academic activity.
Students in clinical activity must upload all immunizations in CastleBranch 4-6 weeks prior to any rotation start so that all required Affiliate Hospital documentation can be submitted within that time frame prior to the start date of clinical activity.
All students who are rotating at the following sites will have their immunizations updated and loaded by the COM Office of Registration and Records.

- All Advocate Sites: Advocate Data Base
This is a requirement for you to participate in COM required academic activity. (Physician Shadowing, DoCS, LPC, Clerkships, Electives, Specialties, All Sub-I’s)
