  • Request an Appointment

    To schedule an appointment with one of our surgeons, call (312) 355-4300.

  • Refer a Patient

    • To refer a patient, physicians can call our surgery call center at (312) 355-4300.
    • Call center hours are Monday through Friday, from 8:00 a.m. to 8:00 p.m.

  • Transfer a Patient

    • To transfer a patient to the University of Illinois Hospital after hours, please call the Illinois Provider Access Line (IPAL) at 855-455-IPAL or 1-855-455-4725.
    • IPAL is a 24/7 physician-only line for physician-to-physician communication, as well as hospital-to-hospital patient transfers. The line’s representatives have access to our physicians’ schedules and telephone and pager numbers to reach the physician promptly.
