Intro

The Department of Pediatrics Division of Population Health focuses on improving access to physical and behavioral healthcare through innovative care models, research, and community partnerships. As part of the department’s mission, the CHECK program serves as its clinical and population health arm.

CHECK provides coordinated medical, behavioral, and social support to a wide variety of individuals and families. Through this program, the division brings its population health strategies to life—improving outcomes and quality of life for some of the most vulnerable patients across the lifespan.
