Apply to Patient-centered Medicine Scholars Program
There are separate applications for each component of the program.
M1 Application
All incoming M1s are eligible to apply to the PCM Program by completing the Program application. A faculty selection committee will review applications and make final selections. Priority will be given to students who have demonstrated social responsibility and commitment to service and patient welfare.
To complete the application, you will be asked to submit the following items electronically:
- 2026-2027 Application Form
- A brief (1- 2 pages) statement describing “Why I want to be a PCM Scholar.”
- Current Curriculum Vitae
Please find information about the M1 curriculum here. Please find the application materials and submit your application following the guidance in your campus assignment email. You can also send any questions you might have to PCMProgram@uic.edu.

M2 Service Learning Program Application
To complete the application, you will be asked to submit the 2026-2027 SLP Application Form

M4 Interprofessional Approaches to Health Disparities Application
To complete the application, you will be asked to submit the following items electronically:
- 2026-2027 Application Form
- A brief (1-2 pages) statement describing “Why I want to participate in IAHD”.
- Current resume
- Please assemble all application materials into a single pdf document and submit electronically to PCMProgram@uic.edu. You can also send any questions you might have to the same email address.
