Resilient Chicago Fund
CONNECT-ED will develop and test an emergency department-to-community platform that identifies social needs, connects patients with health and human services, and tracks referral outcomes. The initiative brings together emergency medicine, UI Health, and community partners to create a more coordinated referral system.
