PREREQUISITES AND PLACEMENT IN THE CURRICULUM:
Students must have completed the M3 Obstetrics and Gynecology clerkship.
PURPOSE:
This elective is for the student who would like to add to the basic knowledge gained from the core clerkship. The student is assigned to the antepartum service . This includes evaluation, presentation, and follow-up of hospitalized patients. It also includes participation in the high-risk outpatient evaluation and obstetrical ultrasound unit.
COMPETENCIES:
Ability to perform normal vaginal delivery; Ability to recognize high risk pregnancy; Understanding of basic treatment for high risk conditions; Particularly hypertension, diabetes, and preeclampsia; Ability to perform antepartum testing (AFI, NST, BPP ); Knowledge of Level 2 Ultrasound; Knowledge of fetal echocardiography; Ability to do proper work-up/examination of high risk pregnancy.
INSTRUCTIONAL FEATURES:
The student will spend time in the outpatient consultation unit as well as in the OB clinic. The student will be exposed to management of complicated pregnancies, obstetric ultrasound, and fetal assessment. The student will be expected to do at least one call per week in the labor and delivery unit as well as following the high-risk patients in labor. All students will participate in daily attending rounds, as well as in all departmental/divisional lectures and conferences. The student will spend 2 weeks in U/S and two in the HR clinic. The high risk clinic consists of the following sessions: Preterm labor, Diabetic (2), Medical complications of pregnancy (2), Substance Abuse, as well as two additional sessions where a variety of high risk conditions including multiple gestation and pregnancies complicated by fetal anomalies are followed. 20 to 30 patients are scheduled per session. Inpatient and outpatient seen/followed generally fall into these categories. The most common inpatient problems are: Preterm labor, Preterm premature rupture of membrane, preeclampsia, complication of multiple gestation, diabetes, hypertensive disorder, pyelonephritis, and asthma. Other medical disorders (i.e . cardiac or renal diseases) are encountered less frequently. Antepartum rounds occur M-F 7:30-9:00. Work rounds occur prior to the attending rounds. The student is expected to participate in the evaluation of the patient and help formulate a treatment plan. The student is then expected to follow the patient throughout his/her rotation and the patient’s hospitalization. There are usually 8-10 patients on services so the student will usually follow 3-4 patients per day. In the outpatient setting, students are advised to try and see the same patients each week. The student is expected to evaluate at least 2 “new” patients during each day. The student is expected to perform vaginal deliveries and scrub on cesarean sections when on call. Observation of amniocentesis and CVS are expected. M4 students attend a weekly ultrasound anomoly conference and the prenatal M & M.
ASSESSMENT:
Global observation. Two comprehensive topic presentations.
