Verification of Medical Degree/Certification Letters
If you have an application form for:

Requests for verification of graduation prior to 1994 require additional documentation that can only be provided by the University of Illinois at Chicago’s Office of Admissions and Records, which may delay our processing of your request.
Copy of Dean’s Letter or Medical Student Performance Evaluation (MSPE)
The College of Medicine Registrar’s Office does upload a copy of Dean’s Letter or Medical Student Performance Evaluation (MSPE) via Medical Institution Document Upload Service (MIDUS) to ERAS.

The College of Medicine Registrar’s Office does not produce or send the University Transcript. The University Transcript have to order from the University Registrar’s Office. You can request a copy of your University Transcript from that office and upload it yourself to ERAS or have them sent directly to the program.

To request a copy of your Dean’s Letter or Medical Student Performance Evaluation (MSPE) be sent to a fellowship program if they do not use ERAS application process, contact either Kathleen Helling, firstname.lastname@example.org, or Kayla Taylor, email@example.com. Please include where a copy of your letter is to be sent to.
Note: Copies of letters cannot be sent directly to the graduate.
