Beginning with Summer/Fall 2013 applications, applicants will no longer submit paper credentials. All applicants are required to upload their documents and academic credentials electronically to satisfy both program-specific requirements and admission/institutional requirements. Under “Degree Admissions “, select “Domestic” or “International”, review the list of required documents, and then select “Apply Online”. Links are provided to view instructions and information. When completing the application, under “Select your Major” chose “PHD GRADUATE EDUCATION IN MEDICAL SCIENCES (GEMS)” from the drop down menu. The Program Code for GEMS is 20FS5375NONE. The last section at the bottom of this page describes how to rank your interest in the 6 PhD granting departments of the GEMS program. You may list up to 3 departments in order of preference with #1 as your first choice.
YOU MUST SELECT AT LEAST ONE OF THE DEPARTMENTS FROM THE LIST BELOW. ( Please note that specifying a particular program indicates your current interest; however, students are free to choose any of the departments for their laboratory rotations and thesis work.)

Within 3 – 5 business days after submitting your online application, you will receive an email acknowledging the receipt of your application, as well as indicating that you may log back into the system to begin uploading application materials. (personal statement, resume, scanned copies of Registrar-issued transcripts and Graduate Appointment form)
Graduate Appointment form (PDF). This document is required for certain pre-matriculation Graduate College Fellowships for which you may be eligible. Please review the guidelines for the University Fellowships and the Abraham Lincoln Graduate Fellowship. Eligible applicants may be nominated by their department of interest.
Personal Statement of Purpose, Relevant Experience and Goals
Request that the Educational Testing Service send GRE and TOEFL (if applicable) scores to the University of Illinois at Chicago. The Institution Code for both GRE and TOEFL scores is 1851.
Additionally, select the Departmental Code for your first choice department for the relevant scores (GRE and TOEFL Department Codes differ while the Institution Code is the same). For GRE score transmission select the appropriate Departmental Code from among the following: Anatomy and Cell Biology, 0201; Biochemistry and Molecular Genetics, 0202; Microbiology and Immunology, 0212; Pathology, 0215; Pharmacology, 0216; Physiology and Biophysics, 0217. For TOEFL score transmission select the appropriate Departmental Code from among the following: Anatomy and Cell Biology, 32; Biochemistry and Molecular Genetics, 34; Microbiology and Immunology, 07; Pathology, 56; Pharmacology, 30; Physiology and Biophysics, 49.
Transcripts: Applicants should not send official paper academic credentials by mail unless the department expressly requests that you send credentials directly to them. You must still upload an unofficial copy electronically to satisfy the institutional requirements.
Official, sealed academic credentials will be requested from applicants admitted to a degree-seeking program. The request for official paper academic credentials will be noted on the official admission letter from the Office of Admissions. Failure to submit official, sealed academic credentials will result in a registration hold after the 10th day of the admission term, but will only affect registration for the second term at UIC.
Three letters of recommendation are required by GEMS.
Application fee: domestic students: $70.00; foreign students: $70.00. With the web application you may choose to pay with a credit card at the time you submit your application on-line, otherwise send a check or money order to the University. If you defer payment you will need to send the fee to the University by check or money order. Under certain circumstances the application fee may be waived, see Application Fee Waiver Program.
