Students who are currently enrolled in classes at UIC are eligible to receive many preventative outpatient services at the Family Medicine Center, (See Services). For those students who also purchase coverage through the CampusCare health benefit plan, the Family Medicine providers will provide comprehensive primary care services for you as described in the Certificate of Coverage (http://www.uic.edu/hsc/campuscare/), as well as manage any necessary specialty care. Questions concerning your CampusCare coverage should be directed to: 312.996.4915. Students not in CampusCare should consult their health insurance policies to determine how to best manage their larger health care needs. Most insurance plans are recognized at the Family Medicine Center where we can offer you complete primary care. All students must present their student ID cards.

Please note: If you are under 18 years old, you must have a consent form signed by your parents or guardians authorizing your treatment. Except for issues related to birth control and STIs, Family Medicine cannot treat anyone under 18 without this consent.
