To Refer a Patient to UI Health
Option 1
For seamless referrals and to ensure continuity of care, please consider utilizing Epic or EpicCare Link.
Option 2
If you do not have access to EpicCare Link, please fax a copy of your order and the patient's demographic information/factsheet to:
