Thank you for your interest in a Volunteer Faculty appointment with the Department of Family Medicine at the University of Illinois at Chicago. Please be aware that UIC College of Medicine guidelines state, in accordance with an agreement by Chicago area deans, that primary faculty appointments for individuals should be limited to one medical school. If you currently hold a faculty appointment at another medical school, please let us know as you may still be eligible for a Lecturer faculty appointment.
To begin the process, please e-mail your current CV to rosalbaa@uic.edu or fax it to 312.996.2579.  After your CV is received, you will be contacted to schedule an appointment with Dr. John Hickner, Department Head, and asked to complete additional paper and online forms. In addition to these forms, you will be required to submit three original letters of recommendations addressed to:

Once processed, you will receive confirmation via e-mail of your appointment. Volunteer Faculty are expected to be involved in an educational component in the Department of Family Medicine, either with medical students (LPC, ECM, preceptor for M3 clerks); in the residency program curriculum scheduled each Wednesday afternoon; or as a speaker for the Grand Rounds lecture series held monthly for DFM faculty and residents.
