PREREQUISITES AND PLACEMENT IN THE CURRICULUM: Must be an M3 student.
PURPOSE: This elective is intended to familiarize the senior medical student with the specialty of occupational and environmental medicine. Issues in the working population including health effects of exposure to hazards in the workplace, work restrictions, disability, worker’s compensation and occupational disease and injury are covered. Health effects of environmental toxicant exposure in both children and adults are also covered.
COMPETENCIES: By the end of this rotation, students would be able to 1) obtain an extensive occupational and environmental history; 2) discuss occupational and environmental diseases in the differential of a patient’s illness; 3) recognize the interaction between medical and occupational issues in determining work restrictions and disability; 4) become familiar with some medico-legal issues such as FMLA, ADA, social security disability, drug testing, and independent medical evaluations; 5) understand the concerns in medical screening and surveillance programs.
INSTRUCTIONAL FEATURES: Over the course of this four-week rotation, the student will be engaged in a variety of learning activities. The activities will take place at John H. Stroger Jr. Hospital of Cook County and UICMC. Half the time will be committed to outpatient clinical care of workers. One half day a week consists of the occupational and environmental medicine conference with includes case presentations, journal club, grand rounds, process talks, epidemiology methods, occupational-toxicology conference and research presentation, on a rotating basis; students will also participate in toxicology rounds for five hours a week. Lectures by attending physicians will be provided on core occupational medicine topics. Approximately one-third of the time will be spent assisting the consult service with either in-patient or telephone consults. Inpatient consults primarily involve acute work-related injuries in StrogerHospital’s Trauma Unit or other consultations requested by the Internal Medicine inpatient services. The student will present a case conference during the last week of the rotation. Students will attend an industrial site visit with an industrial hygienist to evaluate workplace safety issues.
ASSESSMENT: Students receive feedback from attendings with each case presentation. A formal final evaluation will be made at the end of the rotation after the student’s presentation by the attendings and senior resident assigned to consult service. Feedback is ongoing.
