Scheduling an Appointment

The Family Medicine Center operates by appointment only. Appointments can be scheduled in person or by phone at 312.996.2901. When you schedule an appointment, we’ll need to know some basic information:

  • Your name, current address and telephone numbers (2 phone numbers preferred)
  • Your birth date
  • Health insurance information (have your insurance card ready)
  • Your primary care physician or nurse practitioner’s name
  • Specific information about why you need to see a clinician so we can schedule the appropriate amount of time to attend to your medical condition.

 

We ask that you arrive 15 minutes prior to your scheduled appointment time to complete the check-in process. If you are late for your appointment, you may be asked to reschedule your appointment to another day.

Please be sure to bring your insurance and identification cards with you when you come to the Center for your appointment.

All patient balances (such as co-payments, self-payments) are expected at the time of service. For your convenience, we accept cash, checks and credit card transactions. An ATM is available on the 2nd floor of the Outpatient Care Center or at neighborhood ATMs in University Village.

Advanced Access Scheduling: We have a new scheduling system to improve our ability to offer you timely appointments with your PCP. Appointments will now be opening within a day or 2 to better fit your own schedule. Expect to be offered an appointment within a day or so when you call to schedule.

Cancellations

If you find you are unable to keep your appointment, please let us know as soon as possible. We can then reassign your appointment time and make new arrangements for you. As a service to our patients, we have implemented a telephone appointment reminder system to provide you with the courtesy of a timely, efficient reminder of your next scheduled appointment. Please confirm or cancel your appointment by using your telephone keypad and pressing the appropriate key. Patients who make appointments and fail to show up prevent other patients from accessing needed health care services.

Urgent Medical Needs

 If you have an urgent medical needs at any time, day or night, call the Center at 312.966.2901.

During office hours if you have a medically urgent need, select the keypad option #3. We will schedule an appointment that same day or the next as appropriate. We will make every attempt to schedule you with your primary care provider if he/ she is in the center.

After business hours, your emergency calls are taken by an answering service that will notify the family physician on-call. He or she will return your call as soon as possible.
